Long Distance Service Agreements

Customers are generally asked to select a Local Toll / Long Distance (intraLATA) provider and a Long Distance (interLATA and international) provider because of legal requirements. However, Long Distance Calling Plans often distinguish among in-state calling, state to state calling and international calling.

Qwest L D Corp. (QLDC) and Qwest Communications Corporation (QCC) provide Local Toll / Long Distance Service and Long Distance Service, including in-state calling, state to state calling and international calling. Qwest Corporation (QC) provides only Local Toll / Long Distance (intraLATA) service.

The terms and conditions that apply to and govern your Qwest long distance services are set out in tariffs, catalogs, price lists, service agreements, contracts, and various rate and service schedules. Local Services Service Agreements

Local services are provided by Qwest Corporation. Rates, rules, regulations, terms and conditions that govern these services may be in a tariff, a catalog/price list, a rate schedule or unique contract. Tariffs (and in some states for certain services, catalog/price lists) are filed with regulatory agencies, and they set out the rates, terms and conditions that govern the services and the relationship between Qwest and the customer. Catalogs/price lists and rate schedules are contract provisions that set out the rates, terms and conditions that govern the services and the relationship between Qwest and the customer. Customers agree to these provisions by ordering, using, paying for, and not canceling the services offered pursuant to these catalogs or catalog pages. Unique contracts are terms and conditions that apply to the parties and the services covered in the particular contract.

Service Agreements can refer to a unique contract or to the portions of a catalog that set out the general terms and conditions that apply to the services and/or the relationship between Qwest and the customer, such as limits of liability, dispute resolution, billing, payments, indemnification, and the like. Sometimes excerpts or important parts of a catalog are set out separately, for convenience, and called a Notice of Terms or a Service Agreement.
