· Pursuant to the South Dakota Public Utilities Commission’s rules, the rates, charges, terms and conditions for most of Qwest’s South Dakota Local Exchange services can be found in our Exchange and Network Services Catalog No. 1.
· When you order services and products from our Catalog, the rates, charges, terms and conditions set forth in the Catalog apply to those services and products and serve as the contract between you and Qwest. As a user of any of the services or features in the Catalog, you do not need to sign an agreement, but use of Qwest services constitutes acceptance of, and agreement to, the terms and conditions in the Catalog. For residential customers, a summary of certain important provisions in the Catalog are outlined in the Notice of Terms and Conditions for Qwest Local Exchange Services.
· Existing customer contracts for services, term plan agreements, and other specific contracts or agreements with Qwest are not affected and the contract or agreement will continue until it expires.
· Qwest may change the terms and conditions of your services by adding to or making changes to the Catalog and giving you reasonable notice of important changes, such as rate increases, instead of filing them with the Commission. You remain free to terminate the Catalogued services at any time, unless you have signed or agreed to a term commitment or contract with Qwest for a specified period of time.
· If you have questions about your services or products, contract or term agreement, you may contact Qwest Customer Service at 1 800-244-1111 for customers with residential service, or 1 800-603-6000 for business customers. You should review billing of services promptly and advise Qwest of any perceived errors as soon as possible. You may still file complaints regarding Qwest's services or features with the Commission.
