FCC QC NO. 1 - at the Time of Broadband De-Tariffing

This document contains terms and conditions for the following services that are being de-tariffed and removed from Qwest Corporations’s (QC) Tariff FCC No. 1 effective April 15, 2009: Frame Relay; ATM; Metro Optical Ethernet or QMOE® service; Local Area Network Switching Services; Synchronous Service Transport ("SST"); Self Healing Network Services ("SHNS"); Ethernet Ports over SONET on SST and SHNS; GeoMax® service; and HDTV Net service. For customers with existing service as of April 15th, the affected services are being moved from the Tariff to the QC RSS. For customers with existing, affected services under a term, the rates, terms, and conditions initially agreed upon under the Tariff will continue to govern those services for the duration of the term. This document and any applicable contract contain those rates, terms, and conditions. Month-to-month customers for the affected services are subject to the month-to-month rates in the RSS and to the online month-to-month QC Interstate Private Line and Advanced Network Services ("IPLANS") Agreement and applicable service exhibits, and the month-to-month Wholesale Data Services Agreement, which are posted at http://www.qwest.com/legal. Please note that to ensure that no changes were made to terms and conditions, this document is a "snapshot" of the Tariff at the point of de-tariffing and includes services (e.g., DS-1, DS-3, and services slower than 200 Kbps) that were not de-tariffed; their inclusion in this document has no effect whatsoever. New purchases of the affected services (including changes to existing service) will require customers to sign a new contract and the then-current QC RSS will apply.
